What We Do

Your first visit will consist of a comprehensive evaluation explaining your diagnosis and treatment options. Our team of specialists will review your medical history and develop a treatment plan based on our findings.

BEFORE YOUR FIRST VISIT

Prior to coming to your initial visit with us, we highly recommend bringing the following information for a seamless experience:

Patient Registration Forms – Submit secure online patient forms here or download and print patient forms and bring with you to your visit
Referral form from your general dentist (or referring doctor)
Previous X-rays (if applicable)
List of current medications
Dental insurance cards/forms (if applicable)

If a patient is under the age of 18, a parent or legal guardian must accompany them to ALL evaluation and treatment visits.

Please inform our office if you have a medical condition that may be a concern and/or require medication prior to endodontic treatment.
